How is a door threshold seat slot formed into a slab?

Hi. I am in zone 4a. will be ready to pour my slab very soon.

How do I form for a seat pan into concrete so the door threshold will be lowered?

I thought this was a good idea so there is not a tall trip hazard at the entry doors.
