Frost-Protected Shallow Foundations

I’ve read the ASCE Section 32 that mentions using a FPSF with a crawl space, which I assume includes a strip footing. The question is how to detail this? I’m designing for climate zone 5 in an area that only requires vertical insulation extending 12″ below grade. This is easy when detailing a monolithic footing, but how should a stem wall and strip footing be insulated?
