Foundation slab to patio slab – avoiding concrete thermal bridge?

How do you stop a large patio slab from being a huge un-insulated heat sink when it’s attached to a slab on grade foundation?

    The patio should not be attached to the house slab or foundation wall, it should be independent. Partly because of the thermal issues you are asking about, but also so it is unrestrained when it moves seasonally. That's what I think I'm seeing in that picture.

    If it does butt up against the foundation wall, whether it is attached or not, the foundation and any floor slab can be insulated from the interior to reduce any thermal bridging.
