Detail for ICF frost wall to slab with basement door?

We’re having an addition put on our house.  There are no high performing house builders around here so I’m trying to educate my builder strategically in certain spots.  

This one has me stumped though.

We have an ICF frost wall and basement foundation.  We have a door to the outside in the plans for the basement.  
Their plan is to pour the slab to rest on top of the ICF frost wall with no separation or insulation between the slab and the frost wall. That seems like a terrible idea and an easy way for cold to get in; we’re in a cold climate.

    I haven't had that exact situation on a project as I rarely use ICFs but what experience I do have makes me think that would be a good detail. Another detail I've seen is to cut down the interior side of the ICF but leave the exterior in place, and pour the wall and slab at the same time. You ICF manufacturer might have details available.
