You can start by consulting the GBA Product Guide, which lists 8 cabinet makers. Here is the link: Residential Cabinetry.
Barker Cabinets:

They offer some finishes or you can get them unfinished and DIY.
I agree the the recommendation for Barker cabinets. A somewhat similar option is Keystone. http://keystonewood.com/ Keystone sells only to professionals, and the kit you get from Keystone isn't quite as fully prepped. With Barker, 100% of the screw holes are pre-drilled. With Keystone, many are, but there still some you have to measure for and drill, and you don't get instructions for every step--they assume you are a pro and know how to put together a cabinet. The things I like about Keystone are that they are a little more flexible in what you can order, and the end panels are inherently reasonably finished looking, or can be truly finished for a smaller up charge.
